【Abstract】The reasonable participation of flexible employment groups is the key to improve the quality of participation in China's basic medical insurance system. Based on the data of the China Household Income Survey, this paper evaluates the burden of medical insurance contributions for flexible employment groups. The study found that the low-income group with strong medical security needs had the heaviest payment burden, and the proportion of difficult groups with payment burden was the highest. As the income level increases, the contribution burden of flexible workers gradually decreases. With the increase of consumption level from basic living needs to normal living needs, the contribution burden coefficient of flexible employment personnel has decreased significantly, and the payment burden has increased, especially for low-income and some lower-middle-income groups. This paper suggests that different policy arrangements should be given to flexible employment groups with different incomes. In the specific design of the insurance payment policy, the principle of fair burden with reference to the actual income level of individuals should be more reflected, and the policy should be dynamically adjusted with the characteristics of the times and regions.
