【Abstract】Achieving benefit fairness is an important principle in the design of the basic medical security system. From the perspective of public service equalization, this paper analyzes the benefit equity of basic medical insurance for urban and rural residents in China by constructing a theoretical framework of basic medical insurance benefit equity and a two-stage Heckman model, and using the CHNS survey data from 2009~2015. Under the unrestricted group sample, there are significant differences in benefits between groups. The test results after adding restrictions to the model show that there are still significant differences in the fairness of basic medical insurance benefits among different income groups, but the coefficient level is reduced. The results show that there are significant differences in health care between low-income groups, which are not offset by the complementary effects of commercial medical insurance and social medical assistance, and the increase in health care expenditure will reduce individual hospitalization and other expenses, while the high-income group will have more health care expenditure, thus forming a new inequality.
